Amy is a Licensed Clinical Psychologist in the state of Maryland in the USA. She sought my Google Forms services on Fiverr and placed an order with two milestones.
The first milestone was to create a Google Forms template that will automatically create a document from the submitted answers. The second milestone was to use the template to convert all of her paper-based forms to online ones. The Google Forms template was created with Amy's branding and logo.
Google Forms template
The forms themselves were fairly short with just a few questions on each. Google App Script was added to each Google Sheet that was attached to the Google Forms . When the form is submitted a copy is made of a Google Docs template that is populated with the data from the form.
Google Docs template
The new document is automatically placed in a folder. The document is automatically given a filename that includes the name of the person submitting the form. Amy would also be notified that the form has been submitted and document created.
