Gemma Henderson Permanent Cosmetics
Gemma is a permanent makeup technician based in Devon in the United Kingdom. I was approached by her husband on Upwork about the possibility of converting their paper-based patient forms into a more robust digital system.
Patients have to complete several forms and sign many disclaimers regarding previous treatment, health, allergies etc. Basically, Gemma has to make sure she is covered legally before work commences. Another requirement was to be able have a PDF version of the data in a readable format - the amount of questions and answers fed into a spreadsheet makes it difficult to view the data.
Google Form
I set up the five questionnaires on Google Forms. When patients visited the clinic they would be handed a iPad or other tablet to complete the forms. Originally I had used a third-party add-on as part of the process but then I added Google Apps Script to the spreadsheet and upon the trigger of the forms being submitted, a PDF would be created with all of the details. Gemma could then go through these PDFs with the patient to ensure they had completed all of the questions correctly before signing them.
